Inadequate 2017 re-vetting: Domestic violence allegation not identified

No legal proceedings

A review of Metropolitan Police vetting published in 2026 identified Carrick among the cases involving failure: during his 2017 re-vetting, insufficient checks failed to reveal a domestic violence allegation. He therefore continued serving in sensitive positions.

Angiolini Inquiry extended to David Carrick's career and conduct

Investigation

After the 2023 sentence, the government extended the Angiolini Inquiry to examine David Carrick's career, conduct and missed institutional opportunities. In 2026, this part of the public inquiry remained separate from his existing convictions and particularly sought to understand failures in vetting, supervision and the handling of complaints.

IOPC investigations into police handling of four serious reports

Investigation

The IOPC opened four independent investigations into the Metropolitan Police's handling of serious allegations concerning Carrick between 2002 and 2021. The regulator particularly criticised the termination of internal disciplinary procedures when criminal investigations were closed and the removal of his name from certain systems, preventing a cumulative pattern from being recognised.

Use of police status, firearm and identity to intimidate victims

Conviction

Carrick repeatedly used his position as an armed police officer to gain women's trust and then intimidate them, notably telling them that they would not be believed against a police officer.
